On September 15, 2021, at approximately 8:15 a.m. an employee was driving an aerial lift when the bottom of the front basket caught the edge of a B-Box water flow control valve. The employee continued to raise the aerial lift until the B-Box valve broke and the employee was ejected out of the aerial lift basket onto the front edge of the home's concrete slab. The employee fell 6 feet and was hospitalized for a severe neck injury. | |||||
